Cabin fire during a launch rehearsal killed Grissom, White, and Chaffee.
Trajectory
Never lifted off. On 27 January 1967, during a 'plugs-out' integrated test on Launch Complex 34, a flash fire swept the pressurized cabin within seconds. The crew could not open the inward-opening hatch. The disaster forced NASA to fundamentally redesign the Command Module, including a new outward-opening hatch, delaying the program by 21 months but almost certainly saving Apollo's later flights.
